Ready to Get Started?

Follow these simple steps:

Step 1: Complete and Return Forms
There are two forms that need to be completed:

• Medical Questionnaire – This establishes care so we can proceed as your provider. We cannot request your lab work until we receive this form. Once submitted, we will call you to arrange your lab tests.

• Fee Acknowledgment Form – This provides a transparent breakdown of our pricing for various treatment options and any associated costs.

Please fill out both forms and email them to medical@spartatrt.com.

Step 2: Lab Work
Once we receive your completed forms, we’ll call you to order your lab tests. You’ll be responsible for the cost of these initial labs.

 

Step 3: In-House Appointment with Ben, NP
After we receive your lab results (typically within 2-4 business days), we’ll call you to schedule an in-person consultation with Ben, NP. At this appointment, you’ll review your lab results, discuss your symptoms, and explore potential treatment options.
